Accident Analysis Safety Meeting

The purpose of an Accident Analysis is to provide a means for identifying the root cause(s) and developing corrective actions as determined in the analysis. The “WHY Technique” is one process used to find root cause(s) and develop corrective actions for eliminating the root cause(s).
